1.) Signing up is an agreement to pay a one-time, non-refundable $20 set up fee and monthly payment for a period of 12 months. After 12 months, members will be required to register for a renewal to continue benefits.

2.) The credit or debit card on file is in active standing with sufficient funds and will be charged monthly from the date of subscription. Upon client request, business has authorization of a revised payment date for flexible billing.

Business will contact member for any missed payments or insufficient funds for a new card to replace card on file. If member has missed two or more payments, business has the right to pause or terminate membership plan and/or flag or ban client from future membership status.

3.) Wellness Members can cancel or reschedule within 24 to 48 hours of scheduled appointment time without forfeiting membership credit.

However, if late-cancellations become a repeated issue, Member may be subject to forfeit membership credit within rescheduling window, or place membership on pause, and lastly, termination.

4.) As a courtesy, member agrees to notify business within 2-4 weeks prior to cancellation. Business will not charge any fees for cancellation. Cancellation must occur before next draft is due. Any remaining credits on account must be used within 6 months after membership cancellation.

5.) Member agrees to all of our policies.
